In-Home Assessments
We suggest an initial comprehensive assessment of the client within their home environment to determine recommendations for safety, health, quality of life while offering options to maintain the highest level of independence possible.
Family Conferences
Additional support is offered to facilitate communication among family members to create a plan that respects the strengths and limitations of all care providers.
Care Management
Ongoing care management will create solutions to address unique challenges of each client. This may include referring and monitoring home care services, accompaniment and advocacy for medical appointments, coordination of other planning and care needs to support long term wellbeing. For example:
Determining level of care, arranging services, offering housing options, working with community agencies, referrals to attorneys, financial planning, assistance with Medicaid application, caregiver support.
Clinical Supervision
We can provide individual supervision for Geriatric Mental Health Specialist Candidates and Social Workers pursuing licensure.
